Instructions for filling the Online Application Form COMEDK UGET-2016

  • Belongs to Hyderabad - Karnataka Region: Select appropriate option (Yes or No)
  • Name of the District in the Hyderabad - Karnataka Region: Select: Gulbarga/ Yadgir/ Raichur / Koppal/ Bidar/ Bellary. A certificate, confirming the Hyderabad Karnataka region status, issued by the jurisdictional Assistant Commissioner of the corresponding district as required is to be submitted at the time of Counseling. (The certificate in Annexure III must be in the name of the candidate only).
    Note:
    The candidates who have claimed category such as (SC/ST/OBC) of Karnataka should produce a valid certificate at the time of Counseling. If the certificates are not produced or not valid and if the candidates fall under 40% criteria for lower eligibility his / her rank will become invalid and he / she will not be allowed to participate in the counseling process. The candidates who have claimed Minority / Hyderabad Karnataka status should produce a valid certificate at the time of Counseling. If the certificates are not produced or not valid then such of these candidates will not be entitled to select a seat under the reservation claimed.
  • Qualifying Exam: Select the type of qualifying exam from the drop-down menu 2nd PUC / 12th STD / Others. If selected “Others”, please specify the qualifying exam.
  • Status of Qualifying Exam: Select the option from the drop-down menu Completed / Appearing.
  • Month / Year of passing: Select the month and year of passing the qualifying examination if you have passed the qualifying exam.
  • Board of the Qualifying Exam: Select the board of the qualifying examination. If you are from other board that is not listed, select "Others" in the list and fill in your board name in the space provided.
  • Name of the Institution Last Studied: Enter the name of the Institution you have last studied (2nd PUC / 12th Std/Qualifying exam) in the space provided.
  • Place of the Institution Last Studied: Enter the place of the Institution you have last studied (2nd PUC / 12th Std/Qualifying exam) in the space provided.
  • Roll/Reg/Index No. of 10th STD Marks Card: Enter the Roll / Registration/Index number of 10th Standard or SSLC marks card in the space provided.
  • Roll/Reg/Index No. of 12th STD/2nd PUC Marks Card: Enter the Roll / Registration/ Index number of 12th STD/ 2nd PUC /Qualifying Exam marks card in the space provided if you have already passed or completed the qualifying exam.
  • Whether appeared for COMEDK exam in previous years: Select Yes/ No
  • Number of Previous Attempts in COMEDK UGET: Please enter number of previous attempts you have appeared for COMEDK UGET earlier.
  • Status of Earlier COMEDK UGET Appearance: Select the option as per the outcome of your earlier COMEDK attempt from the drop down available in form.
  • Other entrance exams being pursued: Select the other Exams you are going to appear/have appeared in the current year, if any. This data is only for information purpose.
  • Please upload scan image of Applicant’s "Unique ID proof" as Selected in application form.
  • Please upload scanned copies of Applicant’s Photo and Signature
  • Please upload scanned Signature copy of Father/Guardian
  • Please upload scanned Certificate of Declaration by Head of the Institution Last studied
  • NOTE: All uploaded documents should meet the criteria as mentioned in the brochure and website.
  • Test City: Select the Test City from the available list of test cities in the online application form.
  • Applying for: Candidates have the option to apply for “COMEDK” only or Uni-GAUGE only or BOTH COMEDK and Uni-GAUGE.COMEDK UGET 2016 and Uni-GAUGE EMeD 2016 will be held as one combined exam during the same session on 8th May 2016.The counseling, fee and institutions accepting COMEDK and Uni-GAUGE scores are different. Candidates are expected to read the respective brochures and instructions on website before proceeding to apply.
  • Payment details:
    The fee for COMEDK UGET 2016 is Rs. 1200/- (For PCM / PCB) and Rs. 1500 (For PCMB) and the same will not be refunded due to any reason.
  • Payment Mode: Select the payment mode i.e. Online/ Offline
    Online payment Process
    You can opt for online payment and Pay the application fee online through Net Banking, Credit Card and Debit Card. You will be directed to Citrus payment portal after clicking on submit button to complete the payment process.
    Offline payment process
    Offline Payment can be made through Bank Challan at any branch of Axis Bank. You can view the nearest Axis Bank Branch by visiting the below url:https://www.axisbank.com/WebForms/branch-locator.aspx?OffTypeName=branches
    • Select Offline (Challan based) Payment option.
    • Enter Captcha and Submit the application (Ensure to check entered details before submission )
    • On Application view page, there will be a Tab “Challan”
    • Click on the Challan Tab and Download the Pre-printed Challan
    • Take Print out of the Challan
    • Make the Application fees payment in authorized Axis bank branches.
    • Re-Log in to the Application Portal.
    • Upload the Signed/Sealed Challan scanned copy.
    • Enter the Signed/Sealed Challan details.
    • Select the test city preference.
    • Submit the Application.
  • Declaration: The Candidates are expected to go through the declaration and submit the same online after understanding its implication thoroughly.
  • Mandatory Documents to be uploaded along with online Application Form:
    • Candidate’s Unique ID proof: Passport (first & last pages only of unexpired passport) / Aadhar card / E-Aadhar Card / Voter ID / PAN card / Driving License / School / College Photo ID/Certificate with seal and signature of Head of Institution Last Studied(Format in Annexure IV).
    • Candidate’s Photo – As specified
    • Candidate’s Signature : As specified
    • Candidate’s Father / Guardian’s Signature : As specified
    • Challan for Off-line payments (if applicable)
    • Format as available in Annexure IV with Seal and Signature of the Head of the Institution last studied (2nd PUC/12th STD/Qualifying Exam).
    Application forms without any of the above mandatory documents will be rejected.
    Please review all the details before submitting the Application Form, because all the Details given in the application form will be used for further processes and once the Application Form is submitted online, no changes can be made.
    Candidates have to take a print out of the completed application form and get the same attested by the head of institution last studied in. The print out of attested application form should be brought by the candidate at the time of counseling.
